Thank you for choosing Embrace Mental Health. To ensure a smooth and professional experience, please carefully read the following terms and conditions regarding online bookings for appointments.
1. Online Booking Platform
By using the online booking system provided by Embrace Mental Health, you agree to the terms set forth in this agreement. You may book, reschedule, or cancel appointments online via Halaxy online or via email or phone.
2. Appointment Scheduling
- Availability: Appointments are subject to availability. The available times listed online may be updated based on the practitioner's schedule.
- Booking Process: You are required to complete all necessary fields in the booking form, including your name, contact information, and preferred time. Once your appointment is successfully booked, you will receive a confirmation email.
3. Appointment Confirmation and Reminders
- Confirmation: Upon booking, you will receive an email confirming your scheduled session. If you do not receive a confirmation, please contact us directly to ensure your booking is confirmed.
- Reminders: A courtsey reminder email or text will be sent 36 hours prior to your scheduled session.
- Responsibility: While all efforts are made to ensure reminders are sent, it is still the
responsibility of the participant to manage their appointments.
4. Payment
- Fees: All services are subject to the fees outlined on the website www.embracemh.com.au. Payment is required immediately after the session, unless other arrangements have been made (i.e. NDIS Plan Manager details are provided, or another party is managing payments).
- Non-Refundable: Payments are non-refundable except in cases where the provider cancels the session.
5. Cancellation and Rescheduling Policy
- Cancellation: If you need to cancel or reschedule your appointment, please provide at least 48 hours notice. Cancellations made less than 48 hours prior to the scheduled session may incur a late cancellation fee of the full fee amount.
- Rescheduling: You may reschedule your appointment as long as it is within 48 hours of the original appointment.
- No Show Policy: If you do not attend the scheduled session without prior notice, you will be charged a late payment of the full fee for the missed appointment.
6. Telehealth Sessions
- Platform: If you are booking a telehealth session, it will take place via Zoom. You will receive a link to the session once your appointment is confirmed.
- Technical Issues: If you experience technical issues prior to or during the session, please contact us immediately. We will make reasonable efforts to reschedule or resolve the issue.
- Confidentiality: All sessions, whether in-person or online, will adhere to confidentiality guidelines outlined in the practice’s privacy policy.
7. Confidentiality and Privacy
We take your privacy seriously. Your personal information is stored securely in accordance with applicable privacy laws in Australia, Privacy Act 1988. By booking an appointment, you consent to the collection and use of your personal information for the purpose of scheduling and providing therapeutic services.
8. Client Responsibilities
- Accurate Information: It is your responsibility to ensure the information provided during the booking process is accurate and up-to-date.
- Communication: You agree to communicate any changes to your health, circumstances, or contact details that may affect the services you receive.
- Conduct: You agree to engage in sessions in a respectful manner. Inappropriate behavior or abuse towards the social worker may result in immediate termination of services.
9. Limitation of Liability
While we make every effort to provide high-quality services, Embrace Mental Health and its practitioners are not liable for any direct, indirect, or consequential damages arising from the use of the online booking system, cancellation policy, or services provided.
10. Changes to Terms and Conditions
Embrace Mental Health reserves the right to update or modify these terms and conditions at any time. You will be notified of any significant changes via email or through the online booking system.
